What’s on the MBLEx? Content Areas and Exam Structure Explained

The MBLEx (Massage & Bodywork Licensing Examination) is a 100 question, computer based test administered by the Federation of State Massage Therapy Boards (FSMTB) through Pearson VUE test centers. Candidates get 110 minutes to answer all 100 questions, inside a two hour test center appointment that also includes a security agreement and a short information survey. Per FSMTB’s current MBLEx Content Outline (effective July 2023), the exam draws from seven content domains, ranging from Anatomy & Physiology at 11% of the exam to Client Assessment, Reassessment & Treatment Planning at 17%, the single largest domain. Results are reported as pass or fail only; FSMTB reports results as pass or fail, with no numeric score provided. Most states require a passing MBLEx result, reported to the state’s massage therapy regulatory board, before a candidate can apply for a license. The Seven MBLEx Content Domains

FSMTB builds the MBLEx from a Job Task Analysis survey of practicing massage and bodywork professionals, conducted roughly every five to seven years. The current outline groups exam content into seven domains, each with an approximate percentage of the 100 question exam. Client Assessment, Reassessment & Treatment Planning carries the most weight, reflecting how much of entry level practice is about gathering information, checking for contraindications, and adapting a session to the client in front of you rather than applying one fixed routine.

Weights are approximate, per FSMTB’s MBLEx Content Outline. FSMTB notes distribution is approximate and content is reviewed on a multi year cycle.

A quick walk through what each domain actually covers: Anatomy & Physiology tests the structure and function of body systems, tissue injury and repair, and concepts of energetic anatomy. Kinesiology covers skeletal muscle components, contraction concepts, proprioceptors, muscle and joint locations and actions, and range of motion. Pathology, Contraindications, Areas of Caution, Special Populations covers recognizing pathologies, site specific and condition specific contraindications, and working safely with special populations. Benefits and Effects of Soft Tissue Manipulation covers the physiological and psychological effects of massage, soft tissue technique types, and hot and cold applications. Client Assessment, Reassessment & Treatment Planning covers session organization, intake and consultation, visual and palpation assessment, and clinical reasoning. Ethics, Boundaries, Laws, Regulations covers professional boundaries, the therapeutic relationship, scope of practice, and confidentiality. Guidelines for Professional Practice covers hygiene, sanitation, safety, body mechanics, draping, and business practices.

Exam Format and Timing

The MBLEx is delivered as a fixed length, computer adaptive test (CAT) at Pearson VUE test centers: you answer questions in the order presented, and the exam adjusts the difficulty of the next question based on whether you answered the current one correctly. All 100 questions must be completed within the 110 minute allotment; FSMTB’s candidate handbook is explicit that not finishing all 100 items within that window results in a failing exam. Every question is multiple choice with a single correct answer. There are no scheduled breaks during the 110 minute exam period unless a break was approved as a testing accommodation in advance.

The exam fee is paid directly to FSMTB when you submit your application, and per FSMTB policy, MBLEx application fees are nonrefundable and nontransferable. If you need to retake the exam, you submit a new application and pay the fee in effect at the time. FSMTB does not itself cap how many times a candidate can attempt the MBLEx, though your state regulatory board may set its own limit, so check with your state board directly.

What to Expect on Test Day

Arrive at least 30 minutes before your scheduled appointment time; arriving late is treated as a no show and forfeits your Authorization to Test. Bring two forms of identification that exactly match the name on your application, since the test center will not admit you without acceptable ID. The testing room is strictly controlled: personal items, phones, notes, and study materials are prohibited and must be stored in a provided locker, and you will complete a digital photograph and biometric check in. Results are typically reported to your designated state regulatory board within five business days of your exam date; if you fail, the test center provides a diagnostic performance summary by content area to guide further study.
