How Much Does the MBLEx Cost? Fees Explained (2026)

The MBLEx costs $265 per attempt, whether it is your first try or a retake, as of September 2026. The Federation of State Massage Therapy Boards (FSMTB) sets this fee directly, and per FSMTB’s Candidate Handbook (effective July 2025), MBLEx application fees are nonrefundable and nontransferable once you submit your application. There is no separate retake fee: if you need to test again, you pay the same $265 application fee that is in effect at the time you reapply, and FSMTB does not require a waiting period between attempts. FSMTB also charges a $40 fee for a Result Transfer Form if you want your official result sent to a state regulatory board beyond the one you named on your original application, or sent to yourself as an unofficial copy. The $265 exam fee covers only the MBLEx itself. State licensure fees, background checks, and any application fees your state massage therapy board charges to process your license are separate costs, billed directly by that state, not by FSMTB. Below is a breakdown of what the fee covers, what a retake costs, and how state licensure fees fit into the total.

What the $265 MBLEx Fee Covers

The $265 fee is what you pay FSMTB when you submit your MBLEx application, and it covers your one scheduled attempt at the exam itself: the 100 question, 110 minute computer adaptive test delivered at a Pearson VUE test center, plus FSMTB’s processing of your application and education verification, which the handbook states typically takes five business days. Once FSMTB approves your application, you receive an Authorization to Test (ATT) by email, which is valid for a set testing window; if that window expires before you test, you must reapply and pay the fee again. Fees must be paid in U.S. dollars by credit card, certified check, or money order made payable to FSMTB. FSMTB does not accept personal checks. Per FSMTB’s own policy, all fees are nonrefundable and nontransferable, and all sales are final; FSMTB states it will make exceptions only in cases of technical complications that FSMTB itself caused. That means a missed test appointment, a canceled application, or a change of plans does not entitle you to a refund or a credit toward a future attempt.

Retaking the MBLEx: Same Fee, No Waiting Period

If you fail the MBLEx or miss your scheduled appointment for any reason, you must submit a new application and pay the application fee in effect at that time before you can test again. As of September 2026, that fee is the same $265 as a first attempt; FSMTB does not charge a separate, discounted, or premium rate for retakes. FSMTB does not require candidates to wait a set number of days or months before reapplying, so in practice you can submit a new application as soon as you are ready. When you reapply, you must also update FSMTB on any changes to your personal information, such as your name or address, and note any new testing accommodation requests. If FSMTB already verified your education for a prior application, that step typically does not need to be repeated unless FSMTB specifically requests it.

Other FSMTB Fees to Know About

Beyond the exam fee itself, two smaller fees come up. Pearson VUE charges a $50 fee if you cancel or reschedule your exam appointment within 60 days of the appointment, per the MBLEx Candidate Handbook, so schedule changes made early are the only free ones. The other is the $40 Result Transfer Form fee. By default, FSMTB sends your official MBLEx result only to the one state regulatory board or agency you named on your application, or to you directly as an unofficial result. If you later need your result sent to an additional state board, for example because you are pursuing licensure in a second state, you submit a Result Transfer Form through your FSMTB candidate account for $40 per transfer. FSMTB will resend a result to the same state at no additional cost for up to three months from the original send date; after three months, resending to that same state requires another completed Result Transfer Form and fee.

State Licensure Fees Are Separate From FSMTB Exam Fees

Passing the MBLEx does not by itself grant you a license or a credential. Once FSMTB sends your passing result to your state regulatory board or agency, typically within five business days of your exam date, you still need to complete that state’s own licensure application, which usually carries its own application fee, and in many states a background check fee. Those state level fees are set and collected by each state massage therapy board, not by FSMTB, and vary from state to state. Because state fees and requirements differ, budget for the $265 MBLEx fee as a separate line item from whatever your state charges to process your massage therapy license, and check your specific state board’s website for its current fee schedule before you apply.

Is the MBLEx fee refundable if I do not test?

No. Per FSMTB’s Candidate Handbook, MBLEx application fees are nonrefundable and nontransferable in all circumstances, including missed appointments and expired Authorizations to Test.

Does the MBLEx retake cost more than the first attempt?

No. A retake costs the same $265 application fee that is in effect at the time you reapply. FSMTB does not charge a separate or higher fee for repeat attempts.
